Upsell Agreement Form

This agreement is being made between For the Love of Art, LLC and the venue identified below. This agreement is to better serve the partnership that we have and aid in the mutual benefit of event services provided by For the Love of Art, LLC. at this venue. Terms of Agreement: For the Love of Art, LLC. will provide entertainment services in the form of guided paint lessons to no less than 5 and no more than 30 people at your establishment for the agreed upon date(s) and time as discussed and contracted for. The cost per person will be in the amount of $47 which is in excess of $10 per painter according to our base price of $37 per painter. The additional $10 charge is to cover the costs of 1 Mimosa (or other non-alcoholic beverage of choice) and Charcuterie. At the conclusion of the event my booking company FareHarbor will process all of the credit cards, deduct fees and then compensate For the Love of Art, LLC. the remaining amount. After this process has taken place, a check will be mailed to you for the difference after the fees ($9.73 per paid attendee in attendance). The check will be initiated through our account with Wells Fargo and processed and mailed by Wells Fargo to you. If we do any other events at the regular price of $37 per person, this agreement/contract does not apply. A new upsell agreement form will be required for each date for which this applies. Our other terms of agreement will continue to be in place at all events regardless of this addendum. Should any customer be unsatisfied with the provided refreshments that are included in the price and demand a refund for unfair/unfit services or products from Middleton Inn, we will be obligated to refund the customer the $10 amount of the upcharge to which the amount scheduled to process to Middleton from For the Love of Art, LLC at the conclusion of the event will be deducted per each customer who obtained refunds for said complaints. All other changes or addendums to this agreement must be done in writing or electronically signed by all parties.

By checking this box I agree to this contract and give permission to For the Love of Art, LLC. to provide services to our mutual guests/customers on the agreed upon date(s) within this venue for the allotted time frame (including set up and clean up). I agree to all of the stipulations and agreements here within. This agreement is binding upon submission. I understand that any and all compensation as agreed upon and outlined in this contract will be disbursed to me by check via mail within 2 weeks from the end of the event(s).
